The type of health insurance depends on Profession and Nationality:
3rd-Country Nationals (non-Schengen)
For 3rd-Country Nationals the costs of the health insurance do not depend on the income,but on age, gender and risk-category. This type of insurance is commercial, either of the 'emergency' or 'complex care' type depending on the visa / residence permit.
Exception: Employees at Czech companies and citizens of Croatia, Macedonia, Montenegro, Serbia, Turkey and Japan with a Czech trade license (živnostenský list) do not pay commercial insurance, but pay into the public healthcare system (see below, at the EU Citizens)

* Most embassies accept emergency health insurance for the first visa (and the stay is shorter than 6 months), others accept only 'complex care' a first visa. Please check with your local embassy, or avoid problems and take the 'complex care' health insurance right away.

** Directors on a business visa, taking out a director's renumeration (as source of regular income demanded by the Ministry of Interior) are ALSO required to pay social / public health contributions on the renumeration next to the Complex Care insurance.
*** 3rd-Country non-working spouse / children in a marriage with an EU Citizen,
may in certain cases be insured using the public care health insurance of the EU partner.
